New Patients

 

Please complete a full health history prior to your first visit. Finishing this history before your appointment will ensure your office visit is spent talking with Steve rather than filling out forms.

What should you expect?
If you are a new patient, please download and complete the new patient forms prior to your visit. If you are unable to do so, please arrive 15 minutes early to complete this paperwork as it is crucial to your treatment.

Please avoid wearing any perfumes or scented lotions as The Acupuncture Works strives to maintain a fragrance-free environment for the comfort of patients with sensitivities.

 

Remember to dress in loose-fitting, comfortable clothing. You are welcome to bring a pair of shorts or other change of clothes to be sure that you are comfortable during your treatment.



Cancellations/No Show Policy:

Please provide 24-hour notice of cancellations. Cancellations made within less than 24 hours and all missed appointments will be charged the full amount of the appointment. If you are running late, please call.

Insurance:
The Acupuncture Works does not bill insurance directly at this time. However,  a detailed receipt will be provided for patients to submit to their respective insurance carriers for reimbursement.
